Prostatectomy

The prostate is a gland located below the bladder and in front of the anus (between the bladder and the penis). This gland secretes a fluid that contains sperm in the male reproductive system, which may be due to prostate cancer, urinary tract disease, enlargement and swollen prostate, or benign prostatic hyperplasia (BPH), requires prostate surgery for removal of all or part of it.

 

Surgical method

 

There are different types of prostate surgery, but all types of prostate surgery mentioned below can be performed under both general and spinal anesthesia, depending on the patient's physical condition and the discretion of the surgeon and anesthesiologist, one of these two methods is used.

  1. Open prostatectomy (open prostate surgery): An incision is made in the skin and the surgeon removes the prostate and extra tissue in it. There are two treatments for open surgery:

A. Radical retropubic prostatectomy: The surgeon makes an incision from the abdomen to the penis. In most cases, the surgeon only removes the prostate itself.
B. Radical perineal: The surgeon makes an incision between the anus and the scrotum. This method is usually chosen when due to certain conditions and diseases, radical retropubic surgery is not possible in the patient. In this case, the surgeon cannot remove the lymph nodes. This surgery takes less time than the previous method but may lead to erection problems in the patient.

  1. Laparoscopy: This treatment is the least invasive and incisional treatment for the treatment of swollen prostate or prostate cancer in men. Laparoscopic radical prostatectomy (LRP) is a surgical procedure that requires making several small incisions of about two centimeters in the body so that the surgeon can insert small and special medical devices into the patient's body. The surgeon uses a thin incision tool with a camera to see inside the body. The benefits of this method include reducing bleeding, reducing pain, reducing the length of hospital stay, and shorter recovery times.
  2. Laser prostate surgery: In this type of laser surgery, benign prostate enlargement in men can be treated without the need for incisions on the skin. The surgeon inserts a small fiber-optic rod into the body through the penis and directs it to the urethra. The surgeon then removes tissue from the prostate that blocks the flow of urine.
  3. Endoscopic surgery: Like laser prostate surgery, endoscopic surgery does not require making an incision in the patient's body, and the surgeon uses a long, narrow tube with the help of light and a lens to remove extra parts of the prostate gland. This tube is inserted through the penis and is less invasive than the previous method.
  4. Urethral dilation:

A. TURP: This surgery is a resection (removal of all or part of a tissue or organ in the body) of the urethra for enlarged prostate in men, which is one of the standard methods for benign and large swollen prostate disease. In this procedure, the surgeon separates the extra parts of the patient's enlarged prostate with the help of a wire loop. These pieces enter the patient's bladder and then are evacuated at the end of this treatment. One of the advantages of this surgical method is that the patient is not hospitalized for more than 2 to 3 days and urinary incontinence, which is one of the most common concerns of patients, is minimized.
B. TUIP: The method of urinary incision is for the prostate. In this procedure, the surgeon makes several small incisions in the prostate and the body of the bladder to dilate the urethra.

Proportionate to the amount of prostate swelling and the patient's condition and the patient's medical record and general health, the treating physician determines which surgical procedure is most appropriate for the patient. Step by step Treatment

After entering the hospital and final admission, you will be taken to the international patients’ ward by the head of the international patients’ ward to get acquainted with the treatment staff and your doctor. There, your specialist doctor will examine you. Your medical record will be created and all information such as the pills you are taking or your treatment history or possible problems you have had in the past will be reviewed and recorded by the medical team. You will then be admitted to be monitored to prepare for the operation. In the meantime, you will be evaluated by a nurse and your blood pressure will be checked and a blood sample will be taken from you to register your blood type and that you will go to the laboratory to check your blood sugar and cholesterol, etc. You will also be examined by a cardiologist and you will have an ECG taken from you and an echocardiogram if necessary. You will then be examined by an anesthesiologist to determine if you can have this surgery. You will be given information about the operation ahead of you and pre-surgery training.

You are taken to the operation room according to a pre-determined time. You will be anesthetized and after a few minutes your surgery begins. Then one of the surgical methods is used on you. After the surgery, you will be taken to the recovery ward to be monitored and your vital signs will be recorded second by second and you will be constantly monitored by the anesthesiologist and surgeon to regain consciousness. Once you regain consciousness, you will either return to your ward or be taken to the intensive care unit as determined by your doctor. Your medication, which was started the day before to prevent infection, will continue. Also, your bleeding will be completely under control and your dressings will be changed regularly. On this day, a urinary catheter will be inserted for you and will be checked and washed regularly. Serum therapy has also been started for you to regain physical strength. After being awakened, you will be examined in the ward by your doctor. If you have been sampled, the sample will be sent to a pathologist. This is done to further examine the prostate tissue for cancer or other features. If necessary, more tests will be taken to ensure your health. During this period, according to the doctor's diagnosis, either a painkiller is used for you, or a pain pump is installed.

Nursing care continues and you are examined daily by your doctor. Your catheter is constantly monitored and rinsed regularly. You will also be given the necessary training to take care of the catheter and how to use it. In addition, other care training will be provided to you by the head of the international patient ward. During these two days, with the help of your nurse, you get out of bed and walk so that your body regains its strength.

If your condition is stable, the necessary training after discharge for home care and proper exercise or diet will be taught by the treatment team. If needed and at the doctor's discretion, you may be monitored for an additional night, otherwise you can be discharged on the same day. Your medications will be adjusted and you will be instructed on how to take these medications at home.

It is better to stay in Iran for a week after discharge to avoid a long trip and to easily access the doctor and be examined if you need to. But if you want, you can return home any day you want from the day after the discharge. We will not leave you alone and whenever you have any questions, you can contact us or even have a video call with your doctor through the head of the department. In addition, your ward manager will be in touch with you in the first week after your discharge to ensure your health.
