Recovery After Total Knee Replacement in Iran
We will answer your questions and guide you.
For years, a simple walk might have felt like a monumental task. The constant ache, the stiffness, and the fear of falling can steal the joy from daily life. But with your knee replacement surgery, that chapter of your story is over. You’ve taken the first, most courageous step toward reclaiming your freedom.
Now, a new journey begins—the journey of recovery. This is your chance to turn a new joint into a new life. At Mortaz Hospital, we don’t just see a patient; we see a partner in this incredible transformation. Our mission is to guide you, every step of the way, with expert care and unwavering support.
This article is more than just an exercise list; it’s your roadmap to rebuilding strength, one small movement at a time.
Phase 1: The First Steps of Freedom (The First 2 Weeks)
The goal of these initial exercises is to reduce swelling, promote blood flow, and gently awaken the muscles around your new knee. Think of it as introducing your body to its powerful new joint.
- Ankle Pumps: While lying down, gently move your foot up and down, as if you’re pressing a gas pedal. This simple action is crucial for preventing blood clots and should be done 10-15 times, several times an hour.

- Static Quad Contractions: Tighten the thigh muscle on your operated leg, pushing your knee straight down into the bed. You’ll feel the muscle contract. Hold for 5 seconds, then relax. Repeat this 10 times, 3-4 times a day.

- Heel Slides: Lie on your back. Slowly slide the heel of your operated leg toward your buttocks, bending your knee as much as is comfortable. Hold the stretch for 5-10 seconds before returning to the starting position. This is your first step towards regaining a full range of motion. Repeat 10-15 times, 3 times a day.

A word from our team: Our specialized physiotherapy team at Mortaz Hospital designs each phase of your recovery with precision. Even if you’re continuing your rehabilitation from your home country, our experts provide personalized guidance to ensure your progress is steady and safe.
Phase 2: Building Momentum (Weeks 2 to 6)
In this phase, you will start building a foundation of strength and stability. Every repetition brings you closer to walking freely again.
- Straight Leg Raises: Lie on your back, keeping your operated leg straight. Slowly and carefully lift the entire leg a few inches off the bed, using your core muscles for stability. Hold for 5 seconds and then lower it gently. Repeat 10-15 times, 2 times a day.

- Seated Knee Extensions: Sit on a sturdy chair. Slowly straighten your operated leg, lifting your foot until the leg is parallel to the floor. Hold for 5 seconds before lowering. This exercise is vital for rebuilding strength in your quadriceps. Repeat 10-15 times.

- Standing Knee Bends: While holding onto a table or countertop for support, slowly bend your operated knee, lifting your foot slightly behind you. This is a crucial step for restoring your walking pattern. Repeat 10-15 times.

Phase 3: Reclaiming Your Life (Beyond 6 Weeks)
Once your knee is stronger, it’s time to incorporate more dynamic activities that will help you return to your favorite pastimes.
- Walking: This is your best friend. Start with short, manageable distances and gradually increase your walking time.
- Aquatic Therapy (Hydrotherapy): Exercising in a pool is an excellent way to strengthen muscles and increase range of motion. The buoyancy of water reduces stress on your new joint, making movements easier and less painful.
- Stationary Bike: The stationary bike is an excellent low-impact way to improve your knee’s flexibility and stamina without putting unnecessary strain on the joint. Our physiotherapists will advise you on the proper seat height and resistance level.

The “Do Not” List: Activities to Avoid for a Smooth Recovery
Your journey is about smart, not aggressive, progress. To protect your new joint and ensure long-term success, you must avoid:
- High-Impact Sports: Activities with jumping, sudden stops, or quick pivots (e.g., basketball, tennis, running) are off-limits for a long period.
- Deep Squats or Kneeling: Avoid bending your knee past a 90-degree angle, especially with weight.
- Lifting Heavy Objects: This can put undue strain on your new joint and surrounding muscles.
- Bouncing During Stretches: Always perform stretches slowly and gently, holding the position without bouncing.
