Posterior cruciate ligament and anterior cruciate ligament in the knee: The cruciate ligament is very important for the stability of the knee, and people with a damaged ligament in their knee often complain that their knee suddenly empties. Therefore, many patients with cruciate ligament rupture can have surgery to treat the injury. PCL injury and rupture is usually caused by a direct blow to the knee in a bent position. The posterior cruciate ligament is the strongest knee ligament, and reconstructive surgery is usually performed when the ligament rupture is complete and function is impaired.

Surgical method

PCL / ACL repair is performed either as the arthroscopic method (closed surgery method) or open surgery method. Arthroscopic procedures are far less invasive than open surgery. Complications after surgery are much less and the length of hospital stay and recovery period will be shorter.

In arthroscopic surgery, the surgeon accesses the ligament through several holes and repairs it. Knee arthroscopy can be performed under general or local anesthesia. After the patient is anesthetized or after the area has been sufficiently anesthetized, the surgeon makes holes to reach the knee joint. These holes are made in specific areas that minimize the risk of damage to nerves, blood vessels, and tendons. Through one of these holes, a camera is inserted into the joint, and through the other holes, small tools can be inserted to fix the knee problem. Patients undergoing knee arthroscopy surgery under local anesthesia can often watch the procedure on a monitor to see what is causing the problem. The time of knee arthroscopy varies depending on what needs to be done. After the operation, a soft bandage is applied around the knee. After this surgery, the patient should start his physiotherapy course in consultation with the surgeon. In most patients, a tube is inserted into the surgical site to drain blood and fluids, called a drain. This drain is removed from the patient's body upon discharge from the hospital.

After entering the hospital and final admission, you will be taken to the international patients’ ward by the head of the international patients’ ward to get acquainted with the treatment staff and your doctor. There, your specialist doctor will examine you. Your medical record will be created and all information such as the pills you are taking or your treatment history or possible problems you have had in the past will be reviewed and recorded by the medical team. Then you go to the imaging department with the accompanying nurse to take radiology and MRI images of your knee. You will then be admitted to be monitored to prepare for the operation. In the meantime, you will be evaluated by a nurse and your blood pressure will be checked and a blood sample will be taken from you to register your blood type and that you will go to the laboratory to check your blood sugar and cholesterol, etc. You will also be examined by a cardiologist and you will have an ECG taken from you and an echocardiogram if necessary. You will then be examined by an anesthesiologist to determine if you can have this surgery. Two types of anesthesia are used for this operation, complete anesthesia or spinal anesthesia, which makes your lower body numb, so that you do not feel pain. The medical team decides what type of anesthesia is suitable for you.

You will receive information about the operation you are about to undergo and pre-surgery training. You will then be taken to the operating room at a pre-arranged time. You will be anesthetized and after a few minutes your surgery begins. After arthroscopy to repair the cruciate ligament of the knee and completion of the surgery, you are taken to the recovery ward to be monitored and your vital signs are recorded second by second. You will be constantly monitored by the anesthesiologist and surgeon to regain consciousness. Once you regain consciousness, you will either return to your ward or be taken to the intensive care ward as determined by your doctor. Your medication, which was started the day before to prevent infection, will continue and your bleeding will be completely under control and your dressings will be changed regularly. During this period, according to the doctor's diagnosis, either a painkiller is used for you, or a pain pump is installed. After being awakened, you will be examined in the ward by your doctor.

Your nursing care continues. At your doctor's discretion, new tests will be taken from you to make sure you are healthy. You will then be examined by your doctor. You are still completely under observation and your dressings are changed regularly. A catheter may be attached to you during your stay so that you do not have trouble urinating and do not get out of bed. On this day, a physiotherapist will visit you along with your doctor. Your doctor and physiotherapist will give you the necessary training. With the help of these two, you will get out of bed and you will be taught how to walk properly with a repaired knee. We make sure you can walk alone and you have seen all the necessary training. Then you go back to bed and rest. If the condition is stable, the catheter will be removed. Also, according to the doctor's diagnosis, the drain inserted in the knee will be removed from you.

All nursing care continues and you are trained by a physiotherapist every day after being examined by your doctor. Then, if your condition is stable, after the necessary training for home care and proper exercise by a physiotherapist, you will be ready to leave the hospital. Here the treating physician will come to you again and the final visit will take place and their important trainings and recommendations will be told to you again. Your medications will be adjusted at home and you will be instructed on how to take these medications.

It is better to stay in Iran for a week after discharge to avoid a long trip and to easily access the doctor and be examined if you need to. But if you want, you can return home any day you want from the day after the discharge. We will not leave you alone and whenever you have any questions, you can contact us or even have a video call with your doctor through the head of the department. In addition, your ward manager will be in touch with you in the first week after your discharge to ensure your health. Remember that you have undergone a heavy operation and you should not put pressure on your new knee and you should also continue physiotherapy in your country.
