Thyroidectomy

It is a surgery in which all or part of the thyroid gland is removed, depending on the type and extent of the complication. This gland secretes hormones that are responsible for controlling the body's metabolism and regulating the heart rate to control the rate at which calories are burned. This surgery is performed for treatments such as thyroid cancer, goiter and hyperthyroidism. This type of surgery uses general anesthesia, and often a surgical incision is made under the skin wrinkles on the front and bottom of the neck, and depending on the type and cause of the surgery, all or part of the thyroid is removed. To prevent fluid and blood from accumulating under the skin and at the site of surgery, a tube is often inserted into the wound that attaches to the drain outside the patient's body, depending on the type of surgery, it can be easily removed one or two days after surgery without the need for anesthesia or transferring the patient to the operating room. In thyroid surgery, the patient is expected to be hospitalized for three to four days.

Surgical method

We have two main types of thyroidectomies:

A. Typical: involves making an incision in the front and bottom of the neck in the skin fold and giving the surgeon direct access to the thyroid gland.

B. Endoscopy: In this procedure, a surgical instrument and a small video camera are guided to the neck using smaller incisions, allowing the surgeon to access the gland.

The choice of surgical method depends on the patient's opinion, the patient's physical condition and finally the surgeon's opinion. In both cases, if all of the thyroid is removed during surgery, the patient needs to be tested and the amount of medication taken daily to replace thyroid hormones so that the body does not malfunction. However, if a part of the thyroid is removed during surgery, it is likely that treatment with medication is not needed. This decision depends on the surgical condition, the cause of the surgery and the patient's condition, and the treating physician makes the final decision according to the patient.

After entering the hospital and final admission, you will be taken to the international patients’ ward by the head of the international patients’ ward to get acquainted with the treatment staff and your doctor. There, your specialist doctor will examine you. Your medical record will be created and all information such as the pills you are taking or your treatment history or possible problems you have had in the past will be reviewed and recorded by the medical team. You will then be admitted to be monitored to prepare for the operation. In the meantime, you will be evaluated by a nurse and your blood pressure will be checked and a blood sample will be taken from you to register your blood type and that you will go to the laboratory to check your blood sugar and cholesterol, etc. You will have an ultrasound of your thyroid. You will also be examined by a cardiologist and you will have an ECG taken from you and an echocardiogram if necessary. You will then be examined by an anesthesiologist to determine if you can have this surgery. You will be given information about the operation and pre-surgery training.

You will then be taken to the operation room at a pre-arranged time. You will be anesthetized and after a few minutes your surgery begins. After thyroidectomy surgery, you will be taken to the recovery ward to be monitored and your vital signs will be recorded second by second and you will be monitored regularly by the anesthesiologist and surgeon to regain consciousness. Once you regain consciousness, you will either return to your ward or be taken to the intensive care unit as determined by your doctor. Your medication, which was started the day before to prevent infection, will continue. One of the most important things that is constantly checked during thyroidectomy is your blood calcium level, which is constantly monitored by the ward nurse, and your bleeding will be completely under control and your dressings will be changed regularly. After being awakened, you will be examined by your doctor in the ward. If necessary, tests are taken to ensure your health. During this period, according to the doctor's diagnosis, either a painkiller is used for you, or a pain pump is installed.

Nursing care continues and you are examined daily by your doctor. Then, if your condition is stable, the necessary training will be provided by the treatment team for home care and proper diet for after discharge. If needed and at the doctor's discretion, you may be monitored for an additional night or two, otherwise you can be discharged on the same day. If the patient needs iodine, he should be monitored by a doctor for about a week and the patient's tests should be monitored to determine the amount of medication to be taken.

It is better to stay in Iran for a week after discharge to avoid a long trip and to easily access the doctor and be examined if you need to. But if you want, you can return home any day you want from the day after the discharge. In addition, your ward manager will be in touch with you in the first week after your discharge to ensure your health.
